They are: HUD-56002 Completion Certificate for Property Improvements HUD-56002-MH Placement Certificate for Manufactured Home HUD-56004 Title I New Loan Reporting Manifest HUD-55014 Warranty for New Manufactured Home Previous editions of these forms are now obsolete and should be replaced. Sample copies of the revised forms are enclosed. Additional copies may be purchased from your nearest Government Printing Office bookstore or by calling the GPO Order and Information Desk at 202-783-3238. Enclosed for your information is a listing of GPO bookstores. Some of the changes to these forms are a result of the revised Title I program regulations that became effective on November 18, 1991. The major changes to the forms are: o Completion Certificate for Property Improvements (HUD-56002) now states that the dealer certification is required only for dealer loans, and not for direct loans. In addition, the dealer is advised that knowingly and materially submitting false information to a lender or HUD can subject the dealer to the imposition of civil money penalties. o Placement Certificate for Manufactured Homes (HUD-56002-MH) now can be used for both dealer and direct loans, with the dealer certification required only for dealer loans. The dealer certification also includes a statement that any permanent foundation being financed with the loan proceeds has been constructed in accordance with HUD's Permanent Foundations Guide for Manufactured Housing. _____________________________________________________________________ 2 o Title I New Loan Reporting Manifest (HUD-56004) now contains a certification that any loans being reported more than 31 days after the date of disbursement are not in default. Previously, lenders had to attach a separate certification to this effect. Other forms used in the Title I program are also being revised.
