These forms will no longer be available from your local HUD Office. If the forms you are using carry dates earlier than those listed above, you should order a new supply from GPO. All previous editions of these forms are now obsolete and should be replaced. Also available from GPO are HUD-92004-F, Request for Verification and Deposit, and HUD-92004-G, Request for Verification of Employment. Lenders are strongly encouraged to use these forms to obtain written verification of the borrower's employment and the source of funds for any required downpayment. Enclosed are copies of an order form for your use in purchasing these forms. Please retain one copy of the form for photocopying. GPO will also accept telephone orders placed in accordance with the instructions on the order form.
