MEMORANDUM FOR:  ALL TITLE I LENDING INSTITUTIONS

SUBJECT:  Additional Forms Available from Government Printing Office
 
     Seven of the forms used in the Title I program are now available for
sale from the U.S. Government Printing Office (GPO).  These forms will no
longer be available from your local HUD Office.  The forms now available
from GPO are:
 
   HUD-637         Title I Claim for Loss                          4-88
   HUD-55013       Dealer/Contractor Application                   8-86
   HUD-55014       Warranty for New Manufactured Home              5-88
   HUD-56001-MH    Credit Application for Manufactured Home       11-86
   HUD-56002       Completion Certificate for Property Improvement 6-87
   HUD-56002-MH    Placement Certificate for Manufactured Home     6-87
   HUD-56004       Title I New Loan Reporting Manifest            10-86
 
     The date in the lower right corner of each form indicates when it was
issued.  If the forms you are using carry dates earlier than those listed
above, you should order a new supply from GPO.  All previous editions of
these forms are now obsolete and should be replaced.
 
     Also available from GPO are HUD-92004-F, Request for Verification and
Deposit, and HUD-92004-G, Request for Verification of Employment.  Lenders
are strongly encouraged to use these forms to obtain written verification
of the borrower's employment and the source of funds for any required
downpayment.
 
     Enclosed are copies of an order form for your use in purchasing these
forms.  Please retain one copy of the form for photocopying.  GPO will also
accept telephone orders placed in accordance with the instructions on the
order form.  Some lenders may find it convenient to purchase forms at one
of the U.S. Government bookstores listed on the back of the order form.
