TO:  ALL HUD APPROVED MORTGAGEES:
SUBJECT:  Purchase of HUD Forms from the U.S. Government Printing
            Office
 
     Attached are copies of a revised order form to be used for the
purchase of certain HUD/VA forms from the Government Printing Office
(GPO)
 
     Please note that seven additional items have been added to the
list of forms which will no longer be available from HUD Field
Offices but which must now be purchased from GPO.  These items are
available at GPO Bookstores now.  The new forms are:
 
Title I
     HUD Form 55013    -  Dealer Contractor Application
     HUD Form 55Ol4    -  Warranty for a New Manufactured Home
     HUD Form 56001-MH -  Credit Application for Manufactured Home
     HUD Form 56002    -  Completion Certificate/Property Improvement
                            Loan
 
Title II
     HUD Form 54111    -  Request for Insurance Endorsement
     HUD Form 92206    -  Assignment Request
     HUD Form 93114    -  Contract Termination/Suspension/Retirement
 
     The order form may be reproduced locally, and photocopies are
acceptable to GPO.  Please retain a copy of the order form for future
use.  Telephone orders will be taken by GPO at (202) 783-3238.  Some
of you may find it more convenient to purchase the forms at the U.S.
Government Bookstores listed on the back of the order form.
