These items are available at GPO Bookstores now. The new forms are: Title I HUD Form 55013 - Dealer Contractor Application HUD Form 55Ol4 - Warranty for a New Manufactured Home HUD Form 56001-MH - Credit Application for Manufactured Home HUD Form 56002 - Completion Certificate/Property Improvement Loan Title II HUD Form 54111 - Request for Insurance Endorsement HUD Form 92206 - Assignment Request HUD Form 93114 - Contract Termination/Suspension/Retirement The order form may be reproduced locally, and photocopies are acceptable to GPO.
