The formats below are used by Responsible Entities to document the determination made for an environmental review. The formats differ based on the level of review.

Format for a Part 58 project Categorically Excluded, Subject (CEST) to the Federal laws and authorities referenced at 24 CFR 58.5.

Format for a Part 58 project that is Categorically Excluded, Not Subject (CENST) to the Federal laws and authorities referenced at 24 CFR 58.5 or exempt from the National Environmental Policy Act (NEPA).

Format for Environmental Assessments (EA)
