As of September 2016, the Office of Hospital Facilities (OHF) has completed a revision to OMB Collection No. 2502-0602, Comprehensive Transactional Forms Supporting FHA’s Section 242 Program. The majority of the documents included in this collection are utilized for closing FHA-insured hospital loans, with additional documents supporting program processes such as application review, asset management, loan modifications, surveys and site inspections, and environmental assessments.
Revised forms are effective immediately. Links are provided below to the final documents located on HUDClips. Any commitments issued on or after October 1, 2016, must utilize the updated loan documents published in this collection.
The revised collection corrects, revises, updates, and supersedes the previous collection of Section 242 documents finalized by OMB on February 18, 2014. Changes do not entail any new, unknown, or substantive policy changes. The revisions, however, are necessary to make outdated forms current and revise general forms to reflect the specific terms of FHA’s Section 242 mortgage insurance program for hospitals, as defined by current regulations (24 CFR Part 242) and policy (Handbook 4615.1). The previous collection also omitted a number of necessary forms and included forms that are not used or no longer in use by OHF. Many of the new forms in this revised collection are based on approved Section 232 Residential Care Facility or Multifamily forms that have been modified to specify the information required for the underwriting, construction, and initial/final endorsement project phases of Section 242 hospital projects. New consolidated forms have been created to streamline information requests that previously required multiple documents.
The revised collection removes the following forms from the former 2502-0602 collection: HUD-2-OHF, HUD-3305-OHF, HUD-41901-OHF, HUD-92010-OHF, HUD-92421-OHF, HUD-92432-OHF, HUD-92447-OHF, HUD-92450-CA-OHF, HUD-92451-OHF, HUD-92453-NH-OHF, HUD-92457-OHF. These forms were either not needed for the Section 242 Program, were internal forms that do not require input from external parties, or have been superseded by one of the forms included in this revision.
Certain form numbers were renamed in the revised collection to reflect a consistent naming convention (all OHF forms now have the HUD-9 prefix):
