Alternate Resident Survey for MTW Sites

MTW sites may develop an alternate resident survey to the Resident Survey portion of the PHAS system. The alternate Resident Survey must be submitted to and approved by HUD. MTW sites that are currently using approved alternate Resident Surveys include Cambridge Housing Authority, Keene Housing Authority and Atlanta Housing Authority. Samples of the resident surveys currently being used are below:
