Overview
PHAs
are required to use the template files to create their electronic
5-Year, Annual Plans, and PHDEP Plans from the PHA Plans web site.
The PHA, small PHA, and PHDEP Plans templates are provided below
in four different file formats, including Microsoft Word version
6.0, WordPerfect and the Rich Text Format. The PDF template is available
for users to download and print the template plans only. PHAs should
not make edits to the PDF files electronically.
When Are the Next Five-Year PHA Plans (Standard or Streamlined)
due?
- The
first 5-Year/Annual PHA Plans were submitted by PHAs in 1999 for
their FY2000 year.

The 5-Year Plan covered the five-year period from a PHA's FY2000
to FY2004.

The next 5-Year Plan is due 75 days prior to the PHA's 2005 FY.

- Standard
PHA Plan Template (HUD-50075)
The PHA Plan Template 50075 should be submitted by PHAs with more
than 250 Public Housing units. This requirement is irrespective
of the number of Housing Choice Vouchers that the PHA may also
administer. High Performer PHAs with more than 250 Public Housing
units may submit the 5-Year/ Annual Streamlined Plan 50075-SF
(when 5-Year Plans are due) or the Streamlined Annual Plan 50075-SA.

Streamlined
PHA Plan Template (
HUD-50075-SF and
HUD-50075-SA)
The 5-Year/Annual
Streamlined Plan 50075-SF and the Streamlined Annual Plan 50075-SA
should be submitted by non-troubled PHAs with fewer than 250 Public
Housing units. This requirement is irrespective of the number
of Housing Choice Vouchers that the PHA may also administer and
includes PHAs administering only a Housing Choice Voucher program.
High Performer PHAs with more than 250 Public Housing units may
also submit the 5-Year/Annual Streamlined Plan 50075-SF (when
5-Year Plans are due) or the Streamlined Annual Plan 50075-SA.

Guidance in Completing the Templates
Below
are general guidelines for completing the templates below. For more
detailed instructions, please see Section
3 of the PHA Plan Desk Guide and PIH Notice 2003-21 for streamlined
plans. Related information on Capital
Fund tables and guidance is also available.
- Format
The electronic PHA
Plan template is designed in a question-and-answer format to allow
for easy completion. The template contains numerous checkbox and
text “fields.” The PHA may “double-click” on a checkbox, change
the default value to “checked,” and then click “OK.” This action
will place an “X” in the checkbox automatically. PHAs can also
type a response to a question by clicking on text fields and typing.
- Instructions
Instructions for each component are in the template. PHAs should
respond to all questions unless instructed otherwise or if the
questions are not applicable to the PHA’s programs (e.g., a Section
8-only PHA would not respond to a question related only to public
housing).
- Applicable
Period
Information contained in the PHA Plan covers the period beginning
with the PHA’s upcoming fiscal year. The PHA can report policies
and activities that they expect to employ but may not yet have
in place.
- Update
References
At each annual submission, the PHA should update references to
both the PHA Fiscal Year and the Federal Fiscal year made throughout
the plan and the attachments.
- Due
Date
The PHA Plan is due 75 days prior to the beginning of the PHA’s
fiscal year start date.

Annual Plan Type
On the first page of the standard Annual Plan, the PHA should
indicate the type of Plan being submitted: Standard, Streamlined
(high performing PHA), or Troubled PHA.
- Executive
Summary of the Annual PHA Plan
The Executive Summary is not a required component of the PHA Plan.
A PHA may, at its own option, include a brief executive summary
of the plan, including highlights of major initiatives and discretionary
policies planned for the coming fiscal year.
- Annual Plan Table of
Contents
The Table of Contents includes a list of all Plan components,
as well as required and optional attachments. The Table of Contents
also includes a comprehensive list of all supporting documents
available for review. Once the Plan template is fully completed,
the PHA should include a page number in the right hand column
for each item listed in the Table of Contents.

Notice
PIH 2003-21 (HA) and the Deregulation
for Small PHAs include information on the need for PHAs eligible
to submit streamlined plans to include information on site-based
waiting lists, Section 8(y) homeownership, and use of Project-Based
Vouchers.
