Overview

Resident Service Delivery Models (RSDM) grants are available to serve either families or the elderly and persons with disabilities.

Funds from Elderly/Disabled RSDM grants may be used for activities such as:

  • Meal service to meet nutritional needs.
  • Housekeeping aid.
  • Transportation.
  • Wellness programs and preventive health education.
  • Case management.
