Overview Resident Service Delivery Models (RSDM) grants
are available to serve either families or the elderly and persons with disabilities.
Funds from Elderly/Disabled RSDM grants may be used for activities such
as: - Meal service to meet nutritional needs.
- Housekeeping
aid.
- Transportation.
- Wellness programs and preventive health education.
- Case
management.
