Need an extension? If your grant is ending, and you'd like to request additional grant funding, get the Fiscal year 2009 instructions for requesting a grant extension. You'll need to complete a form HUD-91186-A, Multifamily Housing Service Coordinator One-Year Budget as part of your extension request. Send your request to your local Field Office Project Manager or other representative.

Line of Credit Control System (LOCCS)

Housing owners obtain their grant funds from LOCCS, through an automated telephone Voice Response System (VRS) or online. Get the LOCCS Instructions for Grantees.

To obtain a user name and password for LOCCS, or to change users, grant recipients must complete a LOCCS Access Authorization, form HUD-27054. Provide this completed form to your local Field Office representative.

Each time users call the VRS and obtain grant funds, they must complete a LOCCS/VRS Payment Voucher, Form HUD 50080-SCMF. Grantees keep a copy of this form for their records and send another to their Field Office representative.

Performance Reports

Grant recipients must submit a Semi-Annual Performance Report, form HUD-92456. All Service Coordinators in HUD assisted multifamily housing, whose positions are paid for by grant funds, Section 8 or PRAC operating funds, residual receipts, or excess income, must submit this report. Financial Status Reports

Grantees must submit a semi-annual Financial Status Report, SF 269-A (short form). This report shows the amount of grant funds used over each six month period, and the relationship of the program's expenditures to the total grant amount received. The two reporting periods are October 1 through March 31 and April 1 to September 30. Grantees submit these reports to their Field office representatives no later than 30 days following the end of each reporting period.
