Each month Neighborhood Networks provides potential funding sources to help consortia develop and improve their programs and services. Featured this month are grants from the Qwest Foundation. Previous listings can be found in the box on the right.

Agency/Organization: Qwest Foundation

Program/Grant Description: Qwest is a global phone and Internet service provider. The Qwest Foundation awards grants to organizations that generate high impact and measurable results through community-based programs. The foundation supports programs and initiatives that build lasting value for Qwest customers and the communities it serves. The grantmaker has identified the following areas of interest:

Community: The foundation supports programs that promote economic growth and build the capacity of organizations to provide services.

Education: The foundation supports innovative early childhood education programs. Additionally, the foundation funds programs that effectively use technology to improve pre K-12 public school instruction.

Workforce and Economic Development: The foundation supports programs that build job-entry skills through school-to-career transition, adult learning, and workforce development, and that develop and deliver technology instruction to improve workforce readiness.

Eligibility: Applicants must be recognized as 501(c)(3) tax-exempt organizations under the Internal Revenue Code.

Application Process: In addition to completing the application located on the Qwest Web site, applicants should include:

 -   A cover letter.
 -   An Internal Revenue Service (IRS) determination letter indicating tax-exempt 501(c)(3) status.
 -   A board of directors list, including contact information and affiliations.
 -   An annual report, if available, or other material summarizing the organization's activities.
 -   The most recent expense budget, including itemized operating revenue for the organization.
 -   The most recent audited financial statements or IRS Form 990.
 -   A list of major corporate and foundation donors for the past two years.
 -   A one-page summary of the organization's three major core programs or activities.
 -   The program or project budget.
