Form: 4128
Used By: HUD Staff
Description: In housing and development projects, it is necessary to assess the environmental impact that such a project may have on the area or the area may have on the project. This form, and its accompanying checklist, document the environmental assessment and its findings.

Request for Release of Funds and Certification
Form: 7015.15
Used By: State, local and Native American governments
Description:After an environmental review has been completed, the entity responsible for that review must complete this form in order for HUD funds to be released. The form must be signed by a Certifying Officer and may include conditions to be met.
- Authority to Use Grant Funds
Form: 7015.16
Used By: HUD Staff or State governments
Description: This form is used to release funds after any special environmental conditions have been met.
