HOPWA National Technical Assistance Funding program grantees are assigned a HUD Grant/Cooperative Agreement Officer (GCAO) who is the contact person between the grantee and HUD. The GCAO works closely with the grantee to ensure that the grant is implemented consistent with the Cooperative Agreement Provisions and the Technical Assistance Delivery Plan (TADP) and that the grantee is in effective communication with Field Offices in whose jurisdiction(s) assistance is being provided.
The Reporting Process
After a proposal is selected, the GCAO and the grantee together develop a Technical Assistance Delivery Plan (TADP), the Cooperative Agreement Provisions, and the Assistance Award/Amendment (form HUD-1044). Each document is signed by the GCAO and the grantee. In addition, during the grant period of performance, each grantee is required to submit scheduled reports.

Technical Assistance Delivery Plan (TADP)
Is based on the grantee's proposed activities and budget. The TADP describes the activities, schedule, budget, and reporting requirements for each grantee.

Cooperative Agreement Provisions
Describe the obligations and milestones required of the grantee.

Quarterly Report
Includes a narrative and a financial statement.
The due dates and format are determined with the GCAO. Quarterly reports are required throughout the period of performance of the grant.

Annual Report
Includes an audited financial statement.
|
|
Final Close-out Report
Includes a narrative and final audited financial statements following close-out procedures provided by the GCAO on behalf of HUD.
