As a HOPWA formula grantee, you are required to enter project and activity information in IDIS. The Guide to Using IDIS for HOPWA Grantees (last updated December, 2005) provides basic instructions on using IDIS to record and monitor grantee activities. The HOPWA completion path you follow in IDIS depends on whether your activity provides facility-based housing, facility-based non-housing (for example, an activity to acquire land for a supportive services facility), scattered-site housing, housing information/resource identification/administration, or supportive services. You select from one of five completion paths depending on your activity.

The screens for each path are provided below:

 -   HOPWA Accomplishment Screen - pdf
 -   HOPWA Facility-Based Housing Screen - pdf
 -   HOPWA Facility-Based Non-Housing Screen - pdf
 -   HOPWA Resource Housing Info/Resource ID/Administration Screen - pdf
 -   HOPWA Scattered-Site Housing Screen - pdf
 -   HOPWA Supportive Services Screen - pdf

IDIS Accomplishment Requirements

The IDIS screen and Annual Progress Report (APR) pages provide similar formats for collecting the number of clients, their demographic characteristics for persons who receive some form of housing assistance, the type of activities carried out and amounts expended by activity, the number and type of housing units provided, as well as information on the type of supportive services and amount spent by type of service (or estimated by type, as necessary). Complete activity data should also be entered in IDIS within 90 days of the end of the program year or at the completion of the activity, whichever comes first. The following information should be provided for each activity within IDIS at the end of the program year.

 -   Activity Accomplishment Reporting (MA04 Screen) - Actual accomplishment data should be entered for each activity, as compared to the proposed accomplishments, on the MA04 screen for all active HOPWA activities and for activities completed in the most recent program year. (Entered into IDIS).
 -   HOPWA Accomplishment Data (HOPWA Path) - Accomplishment data should be entered in the related HOPWA accomplishment screens in IDIS for each active HOPWA activity and for activities completed in the most recent program year.
