Names, unique personal identifying codes and other individual information on documents must be kept confidential, as required by the HOPWA regulation at 24 CFR 574.440. If a site is confidential and should not be public information please indicate that in Exhibit G (1-a) in lieu of the site location. An optional worksheet is attached to the APR to help you and your project sponsors and contracted service providers collect information needed to complete Part 2 of the APR. The worksheets may be used to record information manually or to design a computerized system to store and tabulate the information. The worksheets should not be submitted to HUD with the Annual Progress Report. HUD reserves the right to review the information used to complete this report, except for names and other personal identification.
