The project will consist of 72 efficiency units and 6 one-bedroom units. The facility will include a dining room, a large formal living room, a conference room and a resident library, as well as a commercial kitchen. This development will also serve as the leverage to begin the redevelopment of the twenty (2) acres of land between Heywood Wakefield III and downtown, known as "Rear Main Street".

The creation of the rehabilitated living units and services delivery spaces provides a barrier-free environment, which accommodate the aging in place of each resident and meet changing need over the next 10 or more years. This project is a Phase II. Phase I was awarded in FY 2000.

MINNESOTA

It will meet Uniform Federal Accessibility Standards and state accessibility standards. Renovation of the common areas will include the installation of a new fire sprinkler system, upgrade of the fire alarm system, HVAC system and elevators, new light fixtures, addition of a community room and whirlpool room, and the construction of a 2-story addition to house four new offices (including a home health care office), new congregate dining facility and a service kitchen. The supportive services will be either private pay or county assistance through Becker County Human Services.

Funding for support services will come from Medicaid reimbursement and Social Security Insurance Level II.

Funding for these services will come from Social Security and Medicaid with additional services provided by Nursing Sister's Home Care and New York State funded Able Ride. The owner has applied for funding under the Service Coordinator (SC) NOFA to supplement the existing SC function at the project site.
